Privacy Policy

Erin Brockovich, Inc. and its affiliates (“EBI” or “we” or “our” or “us”) are committed to respecting your privacy. This Privacy Policy is a part of the Terms Of Use and describes the types of information collected by us, what we do with it and with whom we share it. It is your responsibility to review this Privacy Policy carefully, especially before providing any personal information to or through the Community Healthbook™ web site (currently located at (the “Website”).

Please note that the headings contained in this Privacy Policy are inserted for convenience of reference only and shall not in any way define or affect the meaning, construction, or scope of any of the provisions contained in this Privacy Policy.

Information We Collect

Information we collect about you includes, without limitation, your full name, address, telephone number, email address, when you register as a User of the Website and/or utilize any Web Services, and other personally identifiable information, such as protected health information (“PHI”) and other personal and financial information you provide in connection with your use of any Website Services and/or if you order products or services via the Website, including without limitation information you provide in connection with logging an issue, reporting, taking a survey, responding to a questionnaire, or other submissions through the Website.

How Information We Collect is Used

The information collected by us may be combined with other information collected about you and/or other users of, or visitors to, the Website, and/or from other sources (e.g., governmental data), and may be used and disclosed in a number of ways by us, to help us improve the Website, serve our users, partner with others, and for use by Erin in connection with advocating for our communities. Such uses include, without limitation, the following:

Unless we have your permission, we do not sell or rent to third parties any of the personal information that you provide to us through the Website, or authorize any of our suppliers to sell or rent any such personal information, and we do not authorize any of our suppliers to make independent use of your personal information for their own purposes, including but not limited to direct marketing. However, non-personal information, such as aggregated information about guests, users, and your activities and submissions utilizing the Website and/or Web Services and/or purchasing products or services, participating in surveys or otherwise providing information to EBI may be freely utilized by us and/or our affiliates and suppliers. Additionally, you may choose to share your personal information with others on the Website (e.g., to form a collaboration network), at your own risk. To the extent you provide information directly to others (e.g., via Linked Sites), we do not have access to or control the use of such information.

Site Usage Information

Like most web sites, the Website also gathers traffic patterns, site usage information and other aggregated data in order to evaluate our guests’ and users' preferences and the effectiveness of the Website. This aggregate usage data does not identify you individually. We may share anonymous, aggregated statistics about guests to and users of the Website with third-parties, and we may allow third-parties to collect aggregate data through the Website and/or supplier sites accessible from or though the Website.

The Website may use cookies to enhance the browsing and usage experience on the Website. “Cookies” are small files or records that are placed on your computer's hard drive (by us or a third party provider of ours) to collect information about your activities on the Website. The information captured about your activities on the Website will make it possible for us (i) to speed navigation and provide you with custom-tailored content; (ii) to remember information you gave to us so you don't have to re-enter it each time you visit the Website; (iii) to monitor the effectiveness of certain of our materials and content; (iv) to monitor total number of visitors, pages viewed, and banners served on the Website, and (v) enforce restrictions applicable to the Website. If you refuse cookies, your experience on the Website may be affected — for example you may not be able to utilize all of the functions on the Website.

Protecting Our Community

The law may require that we and our suppliers share your personal information and non-personal information pursuant to judicial or governmental subpoenas, warrants or other like orders. If we are required to do so, we will, of course, obey the law. In addition, notwithstanding any term to the contrary in this Privacy Policy, we and our suppliers reserve the right to use and disclose your personal and non-personal information in order to investigate, prevent, or take action regarding illegal activities, suspected fraud, situations involving potential threats to the physical safety of any person, and/or violations of the Terms of Use for the Website as permitted by law.

Children's Privacy

We do not knowingly collect any personally identifiable information from children under the age of thirteen (13) years. The Website is directed to persons who are at least thirteen (13) years of age, and you must be at least 18 years of age to register as a User. If a parent or guardian is made aware that a child under the age of 13 has provided us with personally identifiable information through any point of contact, we ask to be contacted at the Contact address or email address set forth herein and we will use commercially reasonable efforts to delete the information about the child referenced from our files.

Changes in Our Privacy Policy

We reserve the right to change our Privacy Policy from time to time — for example, by altering our policy on sharing personal information, without prior notice to you. Be sure to check this page periodically for updates.
